Government revises Mutuelle contributions to match citizens’ income levels
Rwanda’s government has updated the contribution structure for its community-based health insurance scheme, Mutuelle de Santé, to better align payments with citizens’ income levels and strengthen the program’s long-term sustainability.
A Prime Minister’s Order of February 16, 2026, sets annual contributions per person as follows: Category 1 members pay 4,000 Rwf fully covered by the government; Category 2 pay 3,000 Rwf plus a 1,000 Rwf government subsidy; Category 3 pay 5,000 Rwf; Category 4 pay 8,000 Rwf; and Category 5 pay 20,000 Rwf.
Classification is based on household income data collected by Ministry of Local Government (MINALOC) in collaboration with National Institute of Statistics of Rwanda (NISR), supported by information from Rwanda Revenue Authority(RRA) and Rwanda Social Security Board(RSSB).
Enrollment in Mutuelle has expanded from 7% of the population in 2003 to over 88% in 2025, making it central to universal health coverage, according to RBC. Over the past decade, revenues have grown significantly, although expenditures rose nearly 300% due to expanded services and rising healthcare costs.
The benefits package now includes high-cost treatments such as dialysis, cancer care, kidney transplants, complex orthopedic surgery, hip replacements, and assistive devices for persons with disabilities.
A capitation payment model for health facilities is being piloted to improve efficiency, first launched in Eastern Province in December 2025.
